I lent a Pokémon to someone for 5 mins using the lending system so they could beat a gym, the Pokémon was "returned" to me but the other person still had it also somehow. About 10 minutes later mine disappeared and I had to contact the person to try and get the Pokémon back. He gave it back but he had already evolved it and it was lower level then the one that I had that disappeared.
