Someone lent me 3 pokemon which came with active items, I removed those and used others etc. On return of the pokemon I still have the owners original active items. They either didnt get returned or got duplicated.

Hi there. Sorry for the inconvenience. As stated in our Lending Guide,

4. I borrowed someone my Pokemon that was holding a Life Orb, but the Life Orb didn't come back to me?

This is indeed correct, if a Pokemon is holding an item at the time the borrow timer runs out; the item will be put into the Temp-Owners backpack.

 

Therefore, the items didn't get duplicated. They were simply left in your inventory as they are not automatically returned after the lending trade is over. You can always trade the items back or mail it to the person. If you were curious, the person you traded with is Patzilicious. Let me know if you need anything else.
