You get the silver disc when you present a Pokemon with 150+ IVs to the Task Master. (It's not guaranteed that this will be your reward.) A scientist will ask to see your Pokedex, when you give it to him he will start to generate a Porygon. If I remember correctly you have to return to him in two hours (Real time) to receive it.
