Simple. The title says it all :P. I would like to start at johto, is that possible?

Nope you start in Kanto, then do Johto, then Hoenn.

No you can only start in Kanto then after beat elite 4 go to Jhoto (no coming back to Kanto before beat the 8th gym) then go to hoenn (no coming to kanto/jhoto before beat the elite 4)

I think you can go back into Kanto before the Johto E4, but not for Hoenn.
