There are several possibilities, it could be that the server crashed shortly afterwards and there was a rollback, it could be that it is in your pokebox somewhere and you have to use the search to find it, it could be that instead of throwing the ball you accidentally ran away from it and possibly some other situations. The most likely is tho that there was a rollback shortly afterwards, this things sometimes happen, it is unfortunate but part of the game.

Yes so probably server crashed just after you caught it and that wasn't enough time for the pokemon data to be saved, meaning with the slight rollback you lost your pokemon.
