Missed hit or either innefective one cause the loose of hp via life orb's recoil (earthquake on flying type/t-bolt on ground and cie )

Bulba just says whenever the user uses an attack, not if its successful.

 

Can you show me in the real game, maybe a video of it working so i can see and change it?

On Bulba, it does also say on the descriptions.

An item to be held by a Pokémon. It boosts the power of moves, but at the cost of some HP on each hit.

Oki should be fixed in next update.

Just in case there isn't some confusion with "each hit", multi-hit moves like pin missle, rock blast, etc, the recoil from life orb is always those 10%, regardless of hitting 2 or 5 times.
