(also link doesn't work) what you will need with your report is Screenshot evidence from the ingame chat showing you and said users agreement. a small description of what happened. make sure the screenshot evidence shows everything in which was said regarding the agreement. if you do not have enough evidence/no evidence proving your side of the story staff will not/may not be able to investigate. i suggest using an image host like imgur or google drive which can also host images.
