Ecruteak is the fourth badge, which you've already unlocked/won from the looks of your trainer card. However, you have not yet won the fifth badge, which is obtained from Chuck over at Cianwood City (the city you had to surf to in order to get the Secretpotion for Jasmine's Ampharos at the Lighthouse). Go back to Cianwood and fight Chuck for the fifth badge - he's a Fighting Type gym leader.
