Once you buy the MS Medallion the item goes into your bag. Check the 'Misc Items' tab in your bag and it should be there. Click the medallion to activate it.

Have you tried reinstalling the game?

No you wont get another chance to catch Mew if you release the one you have already. The boss is designed to give players, who lost their chance to catch their legendary/s, another chance to catch them. It was not designed to give a player infinite chances at getting a perfect legendary. I highly recommend that you don't release your Mew.

Check your CPU usage in the task manager. Perhaps you have a program running which is slowing down your PC. Also, in case you wanted to check, you can click F2 in-game to see your FPS.
