Your Poke has been deleveled. You can try to let it faint 1 time, then close your Client for about 30 secs, log back in, heal up and level it up again. Then it should evolve. And be sure to double check since it could happen that one of the happiness Evos (Umbreon at night / Espeon at day) is triggered instead. In that case please let it faint 2 or 3 times to lower its happiness enough and request another delevel. Sadly there is nothing I can do for you now since we are not able to de-evolve Pokes. Our Tools are just not capable of de-evolving.
