If I remember correctly the NPC asks you what IV you want to keep, so sadly it might be your fault yes. Although maybe its a little heads up for you that Manaphy doesnt need Atk in the meta. Id recommend you to reroll the nature to timid and run a special moveset like tail glow, surf, ice beam + a fill move like energy ball.

Its actually the other way around. You get to keep one IV and all other IVs will get rerolled exactly as you described.

Theyve changed it recently. Only if the Pokemons hidden ability is prefered it will have it. Since Goey on Goodra isnt as good as its other abilitys you wont get Goey.
